Family Membership Guidelines

  • Any 2 adults living together may sign up for a family membership
  • Additional individuals added to the family membership must fall under the following guidelines:
    • Dependents under the age of 26 (a dependent is someone that is on the primaries insurance plan)
    • Foster children
    • Legal guardian
    • Foreign exchange student
    • Grandchildren under the age of 18
    • Family members with special needs that are dependent of the primary memeber regardless of age

All memberships require a 30 Day written cancellation notice. 

This plan requires 2 members with a maximum of 10 members.

* This plan requires a minimum number of members including the primary member.
